Your First Visit

Paperwork and forms

Before arriving, complete the new patient paperwork and bring it with you. You may be asked to complete additional paperwork when you arrive.

Arrive 15 minutes before appointment

Enter the building at door #1. Follow the signs and take the elevator or stairs to the lower level. Check in and turn in completed forms when you arrive. 

Parking

Free parking is available in front of the building and in the west parking lot.

What to wear

Wear loose-fitting, comfortable clothing.

Length of first appointment

At your first appointment the intern will complete a thorough intake. The first visit is 1 1/2 hours. Follow-up visits are about 1 hour.

Cancellation policy

Charges will apply to cancellations made with less than 24-hours notice.

Payment

Payment is due at the time of service. We accept cash, check, Visa and MasterCard. We do not accept insurance for payment because we are a teaching clinic.

Interpreter services are available upon request. Contact us before your appointment to make arrangements.
