To complete your financial aid process, you may be requested to provide additional information. The two tables below explain the reason the information is needed for either financial aid documentation or verification.

Citizenship Match: U.S. Citizenship The Social Security Administration did not confirm that you are a U.S. citizen. Submit documentation to verify your citizen status such as your U.S. Passport, Certificate of Naturalization or Birth Certificate.
DHS Match: Eligible Non-Citizen Status The United States Citizenship and Immigration Services (USCIS) of the Department of Homeland Security (DHS) did not confirm that you are an eligible noncitizen. Submit documentation to verify your noncitizen status. Submit documentation to verify your non-citizen status.
Drug Abuse Hold To address issues related to the Anti-Drug Abuse Act of 1988. We cannot take any steps for your financial aid until this is resolved. Contact 202-377-3889 immediately. 
Drug Conviction Resolution: Item 23 on FAFSA

You reported on your FAFSA that you have been convicted of an illegal drug offense. 

Correct this answer or receive assistance by calling 1-800-4-FED-AID.
Potential Overpayment Potential Overpayment of Federal Student Aid Funds. You currently owe an overpayment to the U.S. Department of Education. Contact the Financial Aid Office at finaid@nwhealth.edu for guidance on resolving this condition.
Selective Service Match Selective Service Registration. Selective Service did not verify that you have complied with this requirement. Register online at www.sss.gov or provide documentation that you are exempt from registration.
SSN and DOB Mismatch: Father, Mother, or Parents

In processing your FAFSA, the match of your Father, Mother or Parents' Social Security Number was not completed.

Correct the Social Security Number, Name and Date of Birth at www.fafsa.ed.gov

Student Loan Default Resolution The National Student Loan Data System (NSLDS) indicates that you are in DEFAULT on one or more federal student loans. You are not eligible to receive any federal student aid until you resolve any loan default(s). Once the default is resolved, you may provide documentation to the Financial Aid Office.
Title IV Authorization Allows part of your federal financial aid to cover nonstandard educational costs (including late registration fees, add/drop fees and library fines). Complete the form and sign:

Veterans Status Match The Veteran's Administration (VA) did not confirm that you are an eligible veteran.  Review your response to Items 48 and 49 at www.fafsa.ed.gov. If correct, submit your DD Form 214 (member 4 copy) or other documentation to verify your veteran status.


2014-2015 Financial aid verification

Student IRS Data Retrieval/IRS Tax Return Transcript Verification of 2013 IRS Income Tax Return Information for Student Tax Filers. You (and your spouse) must verify the income tax information submitted on the FAFSA. The best way to verify income is by using the IRS Data Retrieval Tool (IRS DRT) that is part of FAFSA at www.fafsa.ed.gov.

In most cases, no further documentation is needed to verify 2013 IRS income tax return information that was transferred into the student’s FAFSA using the IRS DRT if that information was not changed.

If you (and your spouse) are unable or choose not to use the IRS DRT, they must instead provide a 2013 IRS Tax Return Transcript(s). To order a Tax Return Transcript, visit www.irs.gov.
