Address or name change
- Change your address or name by completing the Employee Address/Name Change Form – available at HRConnection.
Marriage
- Adjust the marital status and number of tax allowances you claim on your W-4 form – available at HRConnection.
- Notify Human Resources of any address or name change by completing the Employee Address/Name ChangeForm – available at HRConnection.
- Designate your spouse as a beneficiary on your life insurance and/or retirement plan
- Add your spouse to medical and dental insurance plans
Birth or adoption of a child
- Adjust the number of tax allowances you claim on your W-4 form – available at HRConnection.
- Add the child to the medical and dental insurance plans
- Designate the child as a beneficiary on your life insurance
Death of a family member
- Remove the family member from medical and dental insurance coverage. Add or change Section 125 Plan and/or Dependent Day Care elections
- Update your beneficiary for your life insurance
- Update your beneficiary for your insurance plan
- Adjust the marital status and number of tax allowances you claim on your W-4 form – available at HRConnection.
- Notify Human Resources of any address or name change by completing the Employee Address/Name Change Form – available at HRConnection.
- Notify Human Resources within 30 days if your dependents were covered under Northwestern’s health insurance
- Adjust the marital status and number of tax allowances you claim on your W-4 form – available at HRConnection.
- Add or change Tax Shelter Medical and/or Dependent Care elections
- Update your beneficiary for your life insurance
- Update your beneficiary for your retirement plan
