The following is a list of questions to consider asking your insurance benefits provider to assist you in obtaining chiropractic and acupuncture care.

Chiropractic Care

• Do I have a deductible or co-pay?
• Is prior authorization or notification required?
• Are there any limits on the number of chiropractic visits or services?
• Are therapies covered? (Ultrasound, electrical stimulation, cold laser, etc.)
• Are X-rays and diagnostic imaging covered?
• Are extra spinal adjustments covered? (Knee, elbow, etc.)
• Do I have coverage for custom molded foot orthotics? (CPT code L3030)
• Do I need a referral from my medical doctor? (Not usually required.)

Acupuncture

Acupuncture at The Natural Care Center is provided by licensed acupuncturists. You must specify the type of provider to obtain correct benefits. The Natural Care Center has five licensed acupuncturists; our acupuncture services are billed under the name of the licensed acupuncturist.

• Do I need a referral from my medical doctor?
• Do I need to notify my insurance that I am receiving this service?
• Does my insurance cover acupuncture for my condition?
• Is acupuncture performed by a doctor of chiropractic covered?
• Do I have a co-pay, or deductible and co-insurance?
• Are there any limits to the number of visits?
• Is prior authorization or notification required?
• Are there diagnosis limitations?

Important information regarding insurance coverage.

Worker’s Compensation - You will need to bring this information to your appointment.

• Report your injury to your employer immediately and fill out appropriate forms.
• If you have already seen another doctor you may need approval to change doctors or clinics.
• Obtain the name of your Worker’s Compensation insurance company, claim number, billing address, adjustor’s name and phone number.
• Note the date of injury.

Auto - You will need to bring this information to your appointment.

• Report your injury to your insurance company immediately and obtain a personal injury claim number. Keep record of your claim number; accident date; and the insurance company’s name and billing address along your adjuster’s name and phone number.
• A referral is required for massage.
