Your First Visit
Paperwork and forms
Before arriving, complete the new patient paperwork and bring it with you. You may be asked to complete additional paperwork when you arrive.
Arrive 5-10 minutes before appointment
Enter the building at door #10. Check-in and turn in completed forms when you arrive.
Parking
Free parking is available in the clinic parking lot outside door #10.
What to wear
Chiropractic and acupuncture patients may want to wear loose-fitting, comfortable clothing. Massage therapy clients can undress to their comfort level. For additional privacy, our therapists use proper draping techniques.
Length of first appointment
At your first appointment the intern will complete a thorough intake.
Chiropractic: 1 hour (follow-up visits are 15-30 mins)
Acupuncture and Chinese medicine: 1 hour and 45 minutes (follow-up visits are 1 1/2 hours)
Massage therapy: 80 mins, which includes 20-min intake and 60-min massage
Nurse practitioner services: 45 mins (follow-up visits are 20 mins)
Cancellation policy
Charges will apply to cancellations made with less than 24-hours notice. This policy does not apply to chiropractic care.
Payment
Payment is due at the time of service. We accept cash, check, Visa and MasterCard. We do not accept insurance for payment because we are a teaching clinic.

Interpreter services are available upon request. Contact us before your appointment to make arrangements.
