To verify to an outside party that you have earned a degree or certificate from Northwestern, submit an online Verification of Enrollment Form. We ask for 5 business days to complete your request.

We are also seeing delays with International mail, including instances where some countries are not accepting mail for certain periods of time.
Request for Incomplete Grade
If there are extenuating circumstances that prevent you from completing a class, you may request a grade of Incomplete (I). Complete an Application for Incomplete Grade.
Please save to your computer before completing this request. Once you have completed the form save again and forward to your instructor.

Change your Name
Steps to change your legal name or update to a preferred first name:
- Complete the appropriate request form below:
*Students requesting a legal name change will be prompted to attach the appropriate legal documentation (see below) on the form.
- Correct error in spelling (attach one of the following documents: Birth Certificate, US Military ID, Passport, U.S. Resident Alien Card, State ID Card
- Change due to marriage (attach Marriage Certificate*; must include both old and new name)
- Change due to divorce (attach Divorce Decree*; must include both old and new name)
- Legal change of name (attach Court Petition *or U.S. Naturalization Certificate*; must include both old and new name)*If your new name is not included
Replacement Diploma or Certificate
If you have lost or damaged your diploma or certificate, you may request a replacement:
- Complete Request for Diploma Replacement or Copy
- Bring in, mail, or fax the form to the Office of the Registrar (main floor, room 114). (See form for address and fax number.)
- The replacement fee is $35 per document. Please use the following link to pay for your replacement order:

Request a Transcript
To order an official transcript(s), please use the following link which will take you to the National Student Clearinghouse (NSC) transcript ordering page: NWHSU/ NSC Transcript Order Form
- Northwestern Health Sciences University has authorized the National Student Clearinghouse to provide transcript ordering via the Web. You can order transcripts using any major credit card. Your card will only be charged after your order has been completed. Please allow 3-5 business days to process the request for an official transcript.
- The site will walk you through placing your order, including delivery options and fees. You can order as many transcripts as you like in a single session. A processing fee will be charged per recipient.
- Order updates will be emailed to you. You can also track your order online.
- Post-baccalaureate students: If you are applying to a medical or dental program, please read the agency instructions careful as additional documentation and/or ids may be required. You will be able to upload documents to your transcript request.
Pricing:
- Cost: $10.00 per electronic transcript ordered; $13.00 per paper transcript ordered for on-campus pick up; $11.15 per paper transcript sent through US Mail. Additional fees will be applied for certified or express mail.
- Refunds: Refunds are considered on a case-by-case basis. Please contact the Office of the Registrar with any refund questions at registrar1@nwhealth.edu.
