Send these forms to send to your scheduled speaker:
- Speaker Guidelines and Approval Process (pdf)
- Proposed Presentation Outline and Objectives Form (pdf)
- Speaker Biography and Consent Form (pdf)
- Obtain a W-9 form for the speaker to fill out
- Submit an Honorarium Form if receiving an honorarium
