Payment options
At NWHSU Bloomington Clinic, we are committed to making care affordable for everyone! Some of our services are covered by insurance, while others require self-pay. Below is some detail to help you learn about payment options.
Insurance
We are in-network with most insurance plans, including Medicare and Medical Assistance, for many of our services. Acupuncture and massage therapy are covered by some plans. Chiropractic care is covered by most plans. Patient out-of-pocket costs vary by the coverage your insurance plan offers. As a courtesy we will attempt to verify your insurance benefits for you, however, you are responsible for understanding your coverage. Call the number on your insurance ID card for specific issues like:
- Deductibles, co-pays or visit limitations
- Coverage for services
- Coverage for therapies or X-rays
- If you have an auto or work-related injury, report it to your insurance company or employer prior to contacting the clinic.
Payment for co-pays and non-covered types of care is due at the time of service. You will be billed for services if you have not met your deductible/co-insurance. We accept cash, check, Visa, MasterCard, Discover and American Express.
Medicare
Northwestern Health Sciences University Bloomington Clinic is a Medicare participating provider and accepts Medicare assignments. Depending on plan coverage, the patient may be responsible for a portion of the covered services.
In general, under the Medicare guidelines for chiropractic care, only the manual adjustment of the spine are covered. All other parts of chiropractic care are considered non-covered services. This includes; the examinations that all patients receive, extremity adjustments, therapies to include; ultrasound, electrical muscle stimulation, or therapeutic exercises.
For maintenance care – ABN form required
Many patients feel they would benefit from a chiropractic maintenance care plan after their condition has been corrected or stabilized. However, Medicare does not provide coverage for maintenance care.
If you decide to receive maintenance care or other services that may not be covered, you will be asked to sign an Advance Beneficiary Notice (ABN) form required by Medicare. This form confirms that you understand Medicare does not provide coverage for these services and that you agree to pay for the services.
Self-Pay
If a patient does not have insurance coverage for services, we have self-payments options. The patient is able to self-pay for services based on our normal and customary fees at the time of service. Some of these services include Acupuncture and Chinese medicine, lab services, massage therapy, and naturopathic medicine. Once insurance benefits have been exhausted, there are self-pay options available for Chiropractic and other services. We strive to offer these services at affordable costs.
For any out of pocket costs to the patient, we accept cash, check, Visa, MasterCard, Discover Card and American Express.
