Application fee
There is a non-refundable $50.00 application fee required with submission of your application.

Request official transcripts
Have your official academic transcripts from all previously attended post-secondary institutions (including colleges, universities, vocational/technical schools and PSEO credits) sent directly to Northwestern Health Sciences University. To request an official transcript, contact the Registrar’s Office at your prior institution(s). Transcripts from any previous graduate-level work are also required. Electronic transcripts should be sent to admit@nwhealth.edu or by mail to:

References
Provide the name, email and mailing address of two references who you have known for at least six months. An email address and mailing address are required, so please have this information available when you apply.

References from science instructors and other healthcare professionals are preferred, provided they know you well. Northwestern will send instructions to these people on how to submit a reference. Relatives are not accepted as references.

Write a 2-Page Essay
Write a 2-page essay, divided into two parts:

  • Part 1: Tell us about your background and what you feel is pertinent to your continued education and future profession.
  • Part 2: Describe the responsibilities and key skills required as a Radiologic Technologist. Based on your research regarding this career, describe what characteristics would make a successful Radiologic Technologist and why you feel you would be a good fit for this program.

Minnesota Immunization Form
Minnesota state law requires us to have a signed immunization form on file for each student:
