Minnesota College Immunization Law

 

Minnesota State Law (M.S. 135A.14) requires proof that all students born after December 31, 1956 and enrolled in a public or private post-secondary school in Minnesota be immunized against diphtheria, tetanus, measles (rubeola, red measles), mumps and rubella (German measles) – allowing for certain specified exemptions.

Medical and conscientious exemptions
If you have a medical and conscientious exemption, you cannot submit the online form. You must print out and complete a hard copy of the Minnesota Dept. of Health Immunization Form (pdf) because you either need to have a physician’s signature or the form notarized.

The completed form can then be submitted via mail to the address listed below or submitted via email to admit@nwhealth.edu.
