Minnesota College Immunization Law
Minnesota State Law (M.S. 135A.14) requires proof that all students born after December 31, 1956 and enrolled in a public or private post-secondary school in Minnesota be immunized against diphtheria, tetanus, measles (rubeola, red measles), mumps and rubella (German measles) – allowing for certain specified exemptions.

Medical and conscientious exemptions
If you have a medical and conscientious exemption, you cannot submit the online form. You must print out and complete a hard copy of the Minnesota Dept. of Health Immunization Form (pdf) because you either need to have a physician’s signature or the form notarized.
Submitting the online form
Checking the certification box on the online form is your electronic signature. When you hit ‘Submit’, your completed form is automatically emailed to the Office of Admissions. A copy of your online immunization form is also sent to your email address.
