Application Deadline: Jan.
15, 2010.

Application Form
Faculty Evaluation & Recommendation Form
FAFSA (Free Application for Federal Student Aid)
Application Process: Turn in your application,
with attachments, to the Student Records and Financial
Services office no later than 4:30 p.m. on the deadline
date (Jan. 15, 2010). Written essays must be emailed to mwarner@nwhealth.edu
by the deadline date (Jan. 15, 2010). Refer to the individual
scholarships to confirm if a written essay is required.
Funding for most of these scholarships is dependent on economic
trends over the past year. Based on this information, the
Accounting Department will inform the Scholarship Committee
in late January as to what scholarships will be offered
and the dollar amounts that will be awarded. In order to give
you something to apply for, a list is provided of possible
scholarships (see list below). If there are particular scholarships
on the list that interest you, make sure all of the criteria
required for that scholarship are included with your application
form. You may submit criteria for more than one scholarship
along with your scholarship application form.
Most of the scholarships require a Faculty Evaluation &
Recommendation Form. A Faculty Evaluation & Recommendation
Form is available online (click on the link above) or may be picked up in the SFRS office. You
may leave this form with a faculty member, who will be responsible
for forwarding the completed form to the SRFS office by
the deadline date (Jan. 15, 2010). One completed form may
be used for multiple scholarships.
NOTE: If you are required to complete a Free
Application for Student Aid (FAFSA) because you are normally
eligible to receive federal student loans, you must have already
filed your 2009/2010 FAFSA prior to submitting a scholarship
application in order to be considered for an award.
Award recipients will be reviewed by the Scholarship Committee
at the end of January 2010. All award recipients will be
notified by e-mail the week of Feb. 1, 2010. Recipients
will be honored at the luncheon scheduled on one of the days
during Chiropractic Homecoming.
If you have any questions about the scholarships or process
for awarding, please contact Marla Warner in the SRFS office
at (952) 888-4777, ext. 440 or mwarner@nwhealth.edu.
The information on individual scholarships and which scholarships
are available is subject to change.
