Election form for currently enrolled College of Chiropractic students

The Bachelor of Science in Human Biology election form below is intended only for students currently enrolled in the College of Chiropractic who:

• Enrolled in the College of Chiropractic in January 2013 or earlier
• Are not planning to withdraw
• Are not being dismissed

You are eligible to submit the election form at any time. A graduation fee of $150 will be billed to your student account upon completion of the degree.

If you do not meet the above criteria:
You must complete the Bachelor of Science application (College of Undergraduate Health Sciences) with the Office of Admissions instead.
